-When the cough is productive we expand our differential to include chronic bronchitis, bronchiectasis and smoking. When diagnosing bronchiectasis the gold standard is diagnosis with a non-contrasted CT scan.
The causes of bronchiectasis are very broad, and can be found in a great review by Dr. Barkers in NEJM. However the broad categories include postinfectious conditions, congenital conditions, immunodeficiency, toxic inhalation and rhematic conditions.
