If you have misplaced or lost your PIV, please email Marcie.Evans@va.gov, Staci.Saunders@va.gov, and medchiefs@ohsu.edu immediately.
Follow these steps to regain VA Access so you can resume patient care:
Visit https://eauth.va.gov/accessva/ and use these instructions to register for an ID.me account.
Once you’ve made your ID.me account, now follow these instructions and visit https://eauth.va.gov/accessva/ to gain a PIV exemption.
Use these instructions on “How to Log In After Receiving a PIV Exemption or Password Reset”
Please notify the Chief On-Call if you’re having to go thru this exemption process. It allows the Chief Resident an opportunity to plan jeopardy in the event there is a technical difficulty that would preclude you from providing patient care.

Treatment Guidelines
Eolas: a centralized medical platform managed by the Antimicrobial Stewardship team at the VA. It contains clinical guidelines for management of common infectious disease syndromes including CAP, cystitis, cellulitis, GN bloodstream infections. Here’s a brief How-To Use Eolas page.

Here are some commonly utilized policies/procedures:
Acute Ischemic Stroke (MCP 11-35) (Appendix A is helpful)
Rapid Response Team (RRT) (SOP 129-14)
New: SICU is now responsible for responding to RRTs on surgical patients.
Best Practice: Overnight surgical teams are often understaffed. NF team should still respond and help out until Surgical Primary can be bedside.
STEMI Procedure (updated 12/01/2025)
VA INPATIENT STEMI PROTOCOL
All teams activate STEMI by calling the AOD (x55424).
AOD will review the call schedule to determine our STEMI status (DIVERT vs. in-house):
In-house STEMI available: activate VA STEMI Pagers and pages on-call cardiology fellow
STEMI Divert: If on VA STEMI DIVERT - AOD will let requesting provider know if we are on DIVERT and provider will follow the STEMI DIVERT protocol.
VA Physician:
VA provider identifies a patient with a potential STEMI.
VA provider will call the AOD (55424) to activate STEMI team.
Notify Bedside nurse and CHARGE nurse that the Cath Lab has been activated so they can prepare patient for transport
For patients located on an inpatient unit other than the ICU, charge nurse will activate RRT (*22).
Order medications as needed.
Cardiology fellow, cath fellow and interventionalist will respond to the page and come to patient bedside.
VA provider should remain at bedside to discuss with cardiologist when they respond.
